What this permit controls

Controls work above ground level where a fall could cause injury — roof access, plant servicing, ladders, EWPs and scaffolds. Enforces fall-prevention and fall-arrest controls before access.

How to use this permit

Eliminate or substitute

Can the work be done at ground level? Pre-fabricated / lowered? Default to elimination before fall-arrest.

Engineering controls

Set up edge protection, MEWP or scaffolding. Inspect and tag. Fall-arrest only when fall prevention not practicable.

Brief and start

Brief crew on controls, dropped-object zone, rescue plan. Verify weather window. Issuer signs.

Monitor weather and crew

Re-check wind speed and conditions on interval. Cease if outside the window. Standby person watches.

Close out

Confirm all crew off the surface, no tools left, equipment removed. Sign close-out.
