What this permit controls

Authorises isolation and lock-out before de-energised electrical work — proving dead, applying locks and tags, and controlling re-energisation. For boards, fixed plant and equipment.

How to use this permit

Identify and isolate

Identify the correct isolation point. Operate the isolator to OFF. Apply personal lockout device and tag.

Test for dead

Use a tested live instrument: confirm live on known-live circuit → test the isolated equipment (must read dead) → re-test on known-live to confirm instrument still working.

Discharge stored energy

For capacitor banks, large drives, UPS systems — follow the discharge procedure before contact.

Work

Each worker applies their own lock to the multi-hasp. Work proceeds. Issuer remains contactable.

Re-energise

All locks removed by their owner. Final inspection. Permit holder authorises re-energisation. Sign close-out.
