What this permit controls

Controls any work that produces heat, sparks or open flame — welding, grinding, cutting, brazing, soldering. Enforces fire-watch, area preparation, gas testing where required, and isolation of combustibles before work begins.

H01Ignition of combustible materials in work area

Outcome if uncontrolled: Structure fire, asset loss, burns.

H02Sparks and slag falling onto crew or material below

Outcome if uncontrolled: Secondary ignition, struck-by burns.

H03Heat radiation onto adjacent gas / fuel storage

Outcome if uncontrolled: Vessel pressure rise, BLEVE, explosion.

H04Fume inhalation from welding / cutting

Outcome if uncontrolled: Metal-fume fever, long-term respiratory disease.

H05Eye injury from arc flash and sparks

Outcome if uncontrolled: Arc eye, retinal burn, projectile injury.

H06Fire watch not maintained post-work

How to use this permit

Clear and inspect

Clear combustibles within the spark radius. Inspect for hidden combustibles in walls, voids, ducts. Set up fire-watch.

Test atmospheres

If enclosed: gas-test for LEL%, O₂% and toxics. Re-test on interval defined in the permit.

Brief and start

Brief crew on emergency response and fire-watch arrangement. Begin work only after issuer signs.

Maintain fire watch

Active fire watch during work AND for the post-work duration on the permit (typically 30–60 minutes).

Close out

Final inspection for smouldering. Restore detection / suppression. Sign close-out.
