How to use this permit
Reference the scaffold design and load rating. Confirm tie points and foundation.
Barricade below the work. Brief other trades to stay clear. Erect by ticketed scaffolders only.
Competent person inspects and tags green (complete and safe). Re-inspect every 30 days or after weather event / alteration.
Once green-tagged, other trades may access the scaffold. Tags revert to yellow during alteration; red when unsafe.
Re-establish exclusion zone for dismantle. Lower components, don't throw. Final inspection.
